Adds a new GitHub issue template intended to collect feedback for “Insiders” features of the GitHub MCP Server.
Changes:
- Introduces a new
.github/ISSUE_TEMPLATE/insiders-feedback.mdtemplate with YAML front matter and basic prompts (version/feature/feedback).

Grammar: "a GitHub MCP Server Insiders feature" should be "an GitHub MCP Server Insiders feature" (or rephrase to "an Insiders feature") because "Insiders" starts with a vowel sound.
